Sealed bids are to be completed on the Equipment Bid Form and must be legible, signed and dated, clearly stating the price being bid, and clearly stating the item number and description of the piece(s) of equipment you are bidding on. Include an address and phone number for point of contact. Sealed bids will be accepted until 2:00 pm on May 22, 2015
