SJHS Student Registration

To register a student at Sheridan Junior High School, parents/guardians must complete the following forms/processes. To request registration paperwork, or for any questions regarding registration, please call the school or email:

Required Information

The following information and forms are required as part of the registration process:

  • Registration form asking for address, DOB, parents' names, phones, addresses and work places, emergency contact info, etc.
  • Registration form asking if the student has had special education or if the parents would like the student to be screened for gifted or special education programs, as well as if there is anything special to address regarding the student.
  • Media permission form
  • Medical information form
  • Registration form for selecting classes
  • Student handbook and agreement form (agreement form signature required)
  • Records request
  • Proof of address
  • Child's birth certificate
  • Immunization records

The following information and forms are provided with the registration paperwork, but are optional to complete:
